Space is Limited so please mail in or bring your registration forms before the first Skill Checks clinic!! You don't want to miss out, so send it in ASAP!!

Necessary Paperwork to have filled out:
1- Contract
2- Registration Form
3- Payment Form
4- Medical Release Form
5- Credit Card Authorization Form
6- 1st Month's Payment
Each girl must attend 2 of 3 Skill Check Clinics
(Will be in September - Please see times and days below for each Age Group)
5 & Under - Saturdays 9/11, 9/18, 9/25 - 9:30-10am
6-9 yr olds - Saturdays 9/11, 9/18, 9/25 - 10-11am
10-14 yr olds - Saturdays 9/11, 9/18, 9/25 - 11-12pm
15-18 yr olds - Sundays 9/12, 9/19, 9/26 - 7-8pm
